Logging Into Your Plansource Account
  • Go to https://plansource.ultipro.com/Login.aspx
  • Username: First letter of first name, full last name, last 4 of SSN, FLATS
  • Example: Jon Snow = JSnow1234FLATS
  • Default Password: Birthdate in 8-digit format (MMDDYYYY)
  • If you have previously logged into your Plansource account, you will use the unique password you created. If you need assistance with re-setting your password or accessing your account please reach out to the HR department.

Gross Earnings
  • This can be calculated by pulling the last paystub from each year you have been continuously employed and scrolling down to the bottom "Pay Summary" section. You will add the values of all Gross earnings under the YTD row.

File Your Claim
  • You are able to complete your claim online here or by calling 1-866-832-2363.
    Available 8:15am to 4:30pm, Monday - Friday, and Saturday between 9am and 1pm. Closed state holidays.
  • You will need your social security number, the accurate employer names, addresses, telephone numbers and dates of employment within the last 18 months, your alien registration number if you are not a US Citizen.
  • You will be asked to select a method of payment: VA Debit Card or Direct Deposit. If you select Direct Deposit, you will need to have your routing number and your account number.
  • If you are filing a claim for unemployment benefits and have been separated from your most recent job because your employer is reducing staff, has closed or you are otherwise unable to report to work because of the affects of the Coronavirus, select Laid Off / Reduction in Force as your reason for separation from this company.
Complete Your Weekly Claim
  • If you have filed an unemployment insurance application (UI), you must then file a weekly claim for each week.
  • VEC recommends you do this on Sunday, Monday, or Tuesday of each week if you remain unemployed and want to be paid for the prior week.
  • You will be asked to select a method of payment: VA Debit Card or Direct Deposit. If you select Direct Deposit, you will need to have your routing number and your account number.
  • You can complete and submit your weekly UI payment request by calling 1-800-897-5630 or online here.
Timeline

North Carolina Unemployment

File Your Claim
  • Create an Online Account: DES.NC.GOV
  • You will need your social security number, the accurate employer names, addresses, telephone numbers and dates of employment within the last 18 months, your alien registration number if you are not a US Citizen.
  • You will need your Social Security Number, information about your most recent employment and pay, work history for the last two years and bank routing and account numbers for direct deposit.
  • If you are filing due to COVID-19, select "coronavirus" as your reason for separation.
Complete Your Weekly Certification
  • A Weekly Certification is a series of yes/no questions that helps determine your eligibility for benefits each week. If you do not complete a Weekly Certification, you will not be considered for payment.
  • TIP: N.C. Executive Order 118 allows some requirement to be waived for people filing for unemployment due to COVID-19. If you are filing due to COVID-19, you may answer 'yes' to this question on the Weekly Certification: Did you look for work?
Timeline
  • After you file your claim, your last employer will be given, by law, 10 days to respond to DES. No payment will be released until after this 10-day period. Tijuana Flats will not be contesting unemployment claims due to COVID-19.

Georgia Department of Labor

File Your Claim
  • If you have worked and earned wages in GA in the past two years, you can file a Georgia Internet claim by clicking here
  • If you have not earned wages in the past two years in GA but earned wages in another state, go to a GDOL career center to obtain information for filing a claim against the proper state and you will be registered for employment services in GA.
  • If you reside outside of GA and have not earned wages in the past two years, contact the State Workforce Agency in your state of residence to file a claim.
  • You will need to provide your Social Security Number, valid/unexpired government-issued picture ID, your alien registration number if you are not a US Citizen, employer-issued separation notice or letter, names/address/dates of employers for the last 18 months and your banking account and routing number for direct deposit. If you do not have a banking account, your payments will be deposited to your GA UI Way2Go Debit MasterCard which works like a standard debit card (no paper checks).
Complete Your Weekly Certification
  • Once you file your claim, you will be required to certify your eligibility for each week you claim benefits. You may claim benefits using the telephone interactive voice response system at 866-598-4164, via the Internet here, or in person at a GDOL Career Center.
  • TIP-All work search requirements mandated by OCGA Section 34-8-195(3)(A) are waived for all claims filed on or after March 14, 2020 per emergency rule.

Florida Reemployment Assistance Office

File Your Claim
  • You can file your claim online here via the CONNECT system here
  • You will need your Social Security Number, Driver's license or State ID number, your employment for the last 18 months including for each employer: Name, address and phone number, first and last day of work, gross earnings (before taxes are taken out) during the listed dates, the reason for separation, FEIN (this is found on any W2 or 1099 Tax form you received).
Return Every Two Weeks to Request Your Benefits
  • After applying for benefits, you need to request benefit payment in CONNECT on a biweekly basis.
  • It is important that you request your benefits within 7 days of your scheduled date regardless of your claim status.
  • DEO has suspended the requirements to register with Employ Florida & provide work search contacts through May 2, 2020.

  • You will receive payments via a Way2Go debit card. This will be issued by mail after the first payment on your claim is made via debit card.
Additional Submission Options:
  • CareerSource locations across the state will be providing paper applications and assisting Floridians with submitting their Reemployment Assistance applications. Visit www.CareerSourceFlorida.com for information and center locations.
  • FedEx is offering free printing and mailing of Reemployment Assistance paper applications at over 100 storefronts across the state.

File Your Claim
  • File an initial claim online here. All unemployment claims must be filed online.
  • You will need the following information when filing - a valid email account - your email address will became your Uplink Username. Address, Social Security Number, Indiana Driver's License or valid ID, date of birth and phone number. Last employer's name, mailing address, phone number, dates of employment and reason you are unemployed, most recent paycheck stub.
Complete Your Weekly Voucher
  • Vouchers must be submitted weekly through the Uplink Filing System.
  • All work search requirements mandated by O.C.G.A. Section 34-8-195(3)(A) are waived for all claims filed on or after March 14, 2020 per emergency rule
Timeline
  • Within 10 days of filing, you will receive a wage transcript and benefits computation form. This does not mean you qualify: it is a statement providing a possible weekly benefit amount and an overall maximum benefit amount should you be eligible.
  • Eligibility will be determined during the first 21 business days if there are no issues with your claim. Tijuana Flats will not be contesting unemployment claims due to COVID-19.
